The Philadelphia Phillies will activate third baseman Placido Polanco from the 15-day disabled list on Monday, according to Matt Gelb of The Philadelphia Inquirer.

To make room for Polanco on the 25-man roster, the Phillies have optioned utility player Hector Luna to Triple-A Lehigh Valley.
